PHP script (or other webapp) to allow users to change their password

Not sure if this is too heavy weight for you or not, but there is a
project called "Usermin" which is related to the webmin project.  Take a
look at: http://www.webmin.com/usermin.html

Much like webmin, it's highly customizable.  I haven't used it in years,
but I remember being very happy with it when I did.
