It really depends on what you are trying to track. If you need to have
something really really granular and enterprise level, you have
bugzilla. If you just need a simple bug tracker to go on your website
for a smallish project, you have for example, Flyspray.

For something in between that can grow with you, RT3 (Request tracker 3)
is one of the best out there IMHO :) Masses of documentation, usually a
nicely managed package on *nix machines, and packed with features ...
