Good residential DSL providers?

Just open a service ticket and complain. I had some rebate problems with them
a couple years back (basically I didn't send in a form because they didn't
charge me, and after the rebate period expired, they charged me; I opened a
ticket, and the service rep basically said 'what?!' and sorted it out
immediately).

They do have more than one form on their website though.
