Wheel mice in X are actually 5 button mice (assuming a normal wheel mouse
and not those feature 9 or 10 buttons).  Buttons 1 and 2 function as
normal, clicking the wheel is 3, and 4 and 5 are for scroll up and down (I
forget which is which).  Most distro's 'mouseconfig' will set them up
correctly, including XF86Config files, and I've never had an issue getting
mine to work.
